  • Copy of executed trade confirmation including:
    • The financial institution placing the trade. If Mainstar Trust facilitates the trade, the broker's applicable commission will be included in the purchase cost.
    • Price the trade should be executed at
      • For Limit Orders (provide fixed share price)
      • Market Order
    • Dollar or share amount of purchase
    • CUSIP number or Stock symbol
